Forms to Submit
All forms are mandatory and need to be turned in by their respective deadlines.
ACCELERATED
CREDIT FORM Please list all accelerated credit you have attained
or are currently enrolled in. This information helps us place you
in suitable summer courses without duplicating credit you may already
have. Please complete the form, and submit it online. *Note- classes do not have to be completed, just attempted so that we do not schedule you for a class you may receive credit for.
FINANCIAL
RESPONSIBILITY FORM This form is your agreement that you understand
you are financially responsible for the two courses you will be
enrolled in as a Pegasus student. Please complete the form, and
submit it online.
IMMUNIZATION & HEALTH FORMS
Every UCF student must supply proof of immunization.
This completed form must be mailed or faxed to Health Services (all
necessary information can be found on the form). Click on the Health
Services section for more information.
ENROLLMENT DEPOSIT FORM
To confirm your enrollment with the freshman class, please submit this form along with the required $200.00 admissions enrollment deposit. This fee is non-refundable and is valid only for the term for which you hae been offered admission. This deposit will be applied toward your tuition at the time of enrollment. If you do not enroll, for any reason, this deposit is non-refundable.
